Advance Directive Document


It is a unilateral and revocable declaration of will by which a person freely, consciously, seriously, and unequivocally expresses their refusal to undergo medical means, treatments, and/or procedures in the event of a terminal illness or when, due to medical reasons, unforeseen circumstances, or force majeure, it becomes impossible to sustain life naturally.

REQUIREMENTS FOR THE PREPARATION OF AN ADVANCE DIRECTIVE DOCUMENT

REGARDING THE APPLICANT:

  • Valid official identification containing photograph, signature, and, if applicable, address
  • CURP (Unique Population Registry Code)
  • Updated tax status certificate (not older than 3 months)
  • Proof of address
  • Birth certificate
  • Marriage certificate (if applicable)
  • Email address
  • Telephone number
  • Occupation

REGARDING THE REPRESENTATIVE:

  • Valid official identification containing photograph, signature, and, if applicable, address
  • CURP
  • Address
  • Date of birth
  • Marital status
  • Email address
  • Telephone number
  • Occupation

NOTES:

According to the Advance Directive Law of the State of Guanajuato, representatives authorized to execute the advance directive document may be:

  • Persons over eighteen years of age;
  • Persons with legal capacity;
  • Persons who have not been convicted of a serious crime;
  • Persons who speak the language of the grantor of the advance directive document.

It is important to mention that when requested by the applicant or the Notary, two witnesses must be present at the granting and sign the advance directive document.
